Patient Portal

The patient portal is NOT for emergencies!! Please call directly to our office or dial 911 if you think you have a life-threatening emergency. You must be 18 years or older to use the portal.

If you are an existing patient, you do not need to re-register to access the portal; this actually creates a duplicate account for you in our system.

FAQ on Portal Use

What is a patient portal?
Using a secure username and password provided to you by our office, you will be able to do the following:
  • Send and receive routine messages
  • Update your demographic profile including your phone, address, and emergency contacts
  • Review lab results or diagnostic imaging results
  • Obtain a copy of your PHR (Personal Health Record)
How do I get a username/password?
If you are using the system for the first time, you will need to contact our office and supply us with an e-mail account; the portal can only be activated if you have an e-mail address we can reference. Our office will supply you with an initial username/password and this can be done in person during office hours, or over the phone. After you log-in once, you may change your password.

I'm locked out of my account, what do I do now?
Please contact our office to unlock your account.

Why can't I find the results of my lab test, or other parts of my chart?
First, the patient portal is not meant to be an full on-line replica of your entire medical chart. For example, we can not attach any faxed documents received by our office to the portal. If you are looking for lab results, please allows several days after drawing of labs to have them posted on the portal. The results will only appear after the physician has received them, reviewed them and signed off on them.

Why am I not receiving any e-mail notification from the office that something has been added to my portal account?
The most common problem with this is that you may be using an e-mail SPAM filter.
Please be sure to allow e-mails from the following address:

Note: New patients can pre-register using the portal, but still will need to fill out the proper forms.

If you are an existing patient, you do not need to re-register to access the portal; this actually creates a duplicate account for you in our system.
