Our office will supply you with an initial username/password and this can be done in person during office hours, or over the phone. After you log-in once, you may change your password. I'm locked out of my account, what do I do now? Please contact our office to unlock your account. Why can't I find the results of my lab test, or other parts of my chart? Note: New patients can pre-register using the portal, but still will need to fill out the proper forms.
